Guardians vs. Rays Prediction, MLB ALCS Game 6 Odds & Best Bets (10/19)

Fact checked at 6:38 AM ET, Oct 19, 2026
Odds as of Oct 19, 6:28 AM ET
Tampa Bay is one win from the World Series. The Rays took Games 4 and 5 in Cleveland, 7-3 and 3-1, to lead the ALCS 3-2, and now they're home at Tropicana Field, where they went 55-26 in the regular season. Cleveland has to win twice in two days, with no off day after the trip from Ohio. After Game 5, manager Stephen Vogt said he didn't yet know his pitching plan, except that Gavin Williams is not an option on short rest. Expect a bullpen game. Slade Cecconi may open, with Tanner Bibee and Joey Cantillo likely to cover innings, and that comes after Cleveland's relievers carried most of the last two games.
Tampa Bay counters with Freddy Peralta, who shut Cleveland out over six innings in Game 2. The Guardians finished the regular season 24th in the majors in runs, and they've scored four runs over the last two games. We see Peralta keeping them quiet again while the top of the Rays' order does just enough against a pitching staff patched together from the bullpen to end the series.
The Rays are -150 on the moneyline and the Guardians +124. Tampa Bay opened at -139. The total is 6.5, Over -112 and Under -108.
First pitch is Monday at 8:08 PM ET at Tropicana Field, a domed park, on TBS and truTV.
Guardians vs. Rays Predictions
- Pick #1: Freddy Peralta Over 13.5 Outs (-150)
- Pick #2: Yandy Diaz Over 1.5 Hits + Runs + RBIs (-117)
PICK #1: Freddy Peralta Over 13.5 Outs (-150)
Getting 14 outs is something Peralta does almost every time out. He has done it in 29 of his 34 starts this year, postseason included, and in 11 of 12 since joining the Rays. Over his last eight starts, going back to Aug. 26, he has recorded 14 or more every time, with four of them lasting six full innings, while going 6-0 with a 0.79 ERA across nine appearances. Five of those starts came at Tropicana Field: a 0.99 ERA, 26 strikeouts and eight walks. Against Cleveland in Game 2 he went six scoreless innings on 99 pitches.
The price is -150, a 60% break-even, and his record over the full season clears it comfortably. This is also the cleanest way to back him. An earned-runs bet would ask him to repeat a two-month run he hasn't matched over the whole season; this one only asks him to get through the fifth inning. Cleveland ranked 24th in runs and 26th in OPS during the regular season. He threw nine pitches in relief in Game 4, but that outing replaced his between-starts bullpen session, and he is expected to pitch without limitations.
The hook is the danger. Kevin Cash used only three relievers in Game 5 and should have everyone available, and in the Division Series he pulled Peralta after 66 pitches, with 14 outs on the board, exactly the number this bet needs. In a clinching game, one rough inning could end his night early. Cleveland also gets its second look at him in six days.
PICK #2: Yandy Diaz Over 1.5 Hits + Runs + RBIs (-117)
Diaz is swinging as well as anyone in the Rays' lineup: 5-for-10 with three extra-base hits over the last two games, after a 6-for-25 start to the postseason, and he has led off in every Rays playoff game. In the ALCS he has cleared this number four times in five tries: three in Game 1, two in Game 3, six in Game 4 and four in Game 5. The one miss was Game 2, an 0-for-4 against Parker Messick.
Jonathan Aranda, right behind him, also has five hits in the last two games, and Junior Caminero hits after Aranda. A Diaz single has a good chance of turning into a run, and that alone gets him to two.
At -117 the break-even is 53.9%. Cleveland's opener choice doesn't matter here: Diaz bats right-handed and is the everyday designated hitter. The Division Series is the warning sign. He got to two only once in three games, and a bullpen game means he'll see a different arm almost every time up.
